Fly into Milan and out of Rome (or the reverse) to avoid backtracking. Give Rome more days because there is more to see, and finish there if you want the big sights as the highlight.

Getting between them: High-speed train, about 3 h to 3 h 15 from Milano Centrale to Roma Termini, with frequent departures. Flights are not faster once you add airport time. Distance about 479 km as the crow flies. Check current timetables.

Is Milan or Rome cheaper?

Rome is cheaper by our mid-range estimate: about €172 per person per day against €240 in Milan. The difference is mainly hotels, which in Milan rise sharply during fashion week, the design fair and trade shows. Eating out is also more expensive in Milan. In Rome, sightseeing tickets for the Colosseum and Vatican are the main extra cost.

How long is the train from Milan to Rome?

About 3 h to 3 h 15 on high-speed trains from Milano Centrale to Roma Termini, with departures roughly every half hour at busy times and two competing operators. Booking a few weeks ahead brings fares down. Florence is on the way, so you can stop for a night and split the journey.

Which is better for a first trip to Italy?

Rome. The Colosseum, Roman Forum, Pantheon, Vatican and Trevi Fountain are the sights most people imagine when they think of Italy, and the food is classic Italian. Milan is a working, business-focused city that rewards repeat visitors and design lovers. With a week, do both by train.

Do I need to book the Last Supper in advance?

Yes. Tickets for Leonardo's Last Supper at Santa Maria delle Grazie are limited to small timed groups and often sell out weeks ahead, especially in spring and autumn. Check the official sales site as soon as your dates are fixed. If it is sold out, guided tours that include entry are sometimes still available at a higher price.
